Police Clearance Form

To apply for a police clearance from the Houston Police Department submit the following documents:

  1. HOUSTON POLICE CLEARANCE APPLICATION FORM/ HOUSTON POLICE CLEARANCE ORDER FORM
  2. COPY OF VALID ID

A photocopy of your valid driver's license, Texas identification card, birth certificate or passport. Expired licenses, identification cards and passports will NOT be accepted.

  1. FINGERPRINTS (If applicable)

If you have ever been arrested by the Houston Police Department, you must include a legible set of fingerprints.

  1. GOVERNMENT FEE

$22.22 per application

  1. SERVICE FEE

$40.00 per application - 3 to 4 business day processing
$60.00 per application - Same day processing
